LIABILITY WAIVER

I certify that I have completed the health history questionnaire and verify that my answers are true and complete. I also understand that if this information changes in any way during my retreat or services, it is my responsibility to notify Vita Vie Retreat, Inc, and that I assume the risk for any changes in my medical condition that might affect my ability to exercise.

I understand that I should consult a physician before beginning any new physical fitness program and that if I choose not to I am doing so at my own risk. I am fully aware of the risks and hazards of participating in this physical fitness retreat. I do not have any conditions or ailments that would harm me by participating in this program. I am participating in this program at my own risk. I hereby agree to be responsible for my own welfare and accept any and all risks of delay, unanticipated events, inconvenience, illness, injury, emotional trauma or death.

Vita Vie Retreat, Inc, their employees, contractors, sub contractors, subsidiaries and affiliates, do not own or operate any entity which provides goods or services for your trip including, for example, lodging facilities, transportation companies, food service providers, equipment suppliers, etc. and as a result are not liable for any negligent or willful act of any other third party.

Vita Vie Retreat, Inc, their employees, contractors, sub contractors, subsidiaries and affiliates are not responsible for theft or any damage that occurs to my personal property or belongings. I understand that I am responsible for all of my personal property and belongings.

I hereby release and discharge Vita Vie Retreat, Inc, their employees, contractors, sub contractors, subsidiaries and affiliates from and against any and all liability from my participation in this fitness retreat and all of the fitness services and activities that I may engage in. I agree that this release shall be legally binding upon me personally and all members of my family.
